Description
Viewed from above into the lesser pelvis of an adult male, the pubococcygeus forms the medial sheet of levator ani, arising from the posterior body of the pubis and adjacent tendinous arch of the obturator fascia, then sweeping posteriorly toward the anococcygeal raphe and coccyx. Flanking it laterally, the iliococcygeus spans from the tendinous arch toward the coccyx and lower sacrum, while puborectalis curves posteromedially to form a sling around the anorectal junction. Posteriorly, the sacrum and coccyx define the midline bony boundary, and the coccygeus (ischiococcygeus) lies posterolateral, running from the ischial spine to the sacrum and coccyx. A central pelvic hiatus interrupts the muscular diaphragm. A superior perspective is the clearest way to appreciate how the pubococcygeus and puborectalis narrow the urogenital hiatus and stabilize the anorectal angle. Small changes in tone here matter. Clinically, this anatomy frames the mechanism of male pelvic floor dysfunction, including post prostatectomy urinary incontinence and defecatory disorders when the levator plate weakens or the puborectalis sling fails to maintain the anorectal kink during increases in intra abdominal pressure.
