Description
Posterior to the male heart, the pulmonary trunk can be followed as it bifurcates into the right and left pulmonary arteries, with early lobar branches (and smaller arterioles) coursing laterally toward their respective hila. From this angle, the left pulmonary artery sits inferior to the aortic arch while the right pulmonary artery runs more horizontally toward the right lung, passing posterior to the ascending aorta and superior vena cava. Inferiorly, the arterial tree lies superior to the left atrium and adjacent to the pulmonary venous entries, while the coronary vessels remain superficial on the epicardial surface. Posterior orientation matters because it matches what you see when the heart is mobilized during thoracic surgery and when interpreting cross-sectional imaging where the pulmonary arteries are traced back from the hila to the main pulmonary artery. This view also helps teach relationships that drive disease patterns: thromboemboli lodging at the pulmonary artery bifurcation (a saddle pulmonary embolus) and congenital variants such as a patent ductus arteriosus connecting the proximal left pulmonary artery to the aortic isthmus. Shortcuts in spatial thinking fail here. The vessels overlap.
