Description
Positioned on the visceral (inferior) surface of the male liver, the quadrate lobe lies anterior and inferior to the porta hepatis, bordered on its right by the gallbladder fossa and on its left by the fissure for the round ligament (fissure for the ligamentum teres hepatis). Medially it approaches the fissure that continues posteriorly toward the ligamentum venosum, while posteriorly it relates to the transverse fissure containing the portal triad. Inferiorly, its peritoneal surface faces the first part of the duodenum and the pyloric region of the stomach. A surgical landmark. Recognition of the quadrate lobe from an inferior position matters because it anchors orientation on the visceral surface during hepatobiliary surgery and cross sectional imaging, where lobar nomenclature based on surface anatomy does not always match Couinaud segments. The gallbladder fossa and fissure for the round ligament bracket the plane used to distinguish functional right and left liver; misreading that plane can lead to wrong sided interpretation of lesions or segmental resection margins. This relationship also frames common operative anatomy in laparoscopic cholecystectomy, where inflammation can distort the gallbladder fossa and adjacent hepatic parenchyma.
