Description
Arising along the lesser curvature near the pylorus, the right gastric vein courses superiorly and medially on the anterior surface of the stomach toward the hepatoduodenal ligament. The fundus lies superolateral, with the body of the stomach inferior to the gastroesophageal junction where the distal esophagus enters. Distally, the pyloric canal narrows to the pylorus and continues into the first part of the duodenum on the right. Short tributary venules drain the antrum and pyloric region into the main venous channel. Attention to the right gastric vein pays off in portal venous anatomy because it commonly drains into the portal vein near its formation from the superior mesenteric and splenic veins, though variants include drainage to the left portal branch or to the common hepatic vein confluence. Small caliber does not mean low consequence. During antrectomy, pyloroplasty, or ligation along the lesser curvature in partial gastrectomy, this vessel and its accompanying right gastric artery become a bleeding point and a landmark within the lesser omentum. Portal hypertension can also recruit this territory into portosystemic collateral pathways, and endoscopic or surgical management of upper GI bleeding benefits from a clear mental map of these venous routes. Use this anterior view for teaching the stomach’s venous drainage in gross anatomy and GI blocks, and for labeling figures in surgical texts covering distal gastrectomy and lymphadenectomy along the lesser curvature. It also fits radiology or hepatology content when explaining portal venous inflow and anatomic variants relevant to CT angiography, venography, or preoperative planning.
