Description
Dominating the right cardiac contour, the right atrium lies superior to the right ventricle, with the acute (inferior) margin formed largely by the right ventricle as it approaches the diaphragmatic surface. The superior vena cava enters the right atrium superiorly and slightly posteriorly, while the inferior vena cava ascends to the atrial floor at the inferior aspect of the image. Anteriorly, the pulmonary trunk arises from the right ventricle and courses superiorly, with the ascending aorta positioned posterior and to the right of the pulmonary outflow at its root. Short segments of the pulmonary veins reach the left atrium posteriorly, but the right surface remains the dominant silhouette. Clear boundaries. Reading the right surface correctly matters when you correlate physical exam findings and cross sectional imaging with chamber enlargement: right atrial dilation can widen the right heart border on a PA chest radiograph, and right ventricular hypertrophy pushes the anterior cardiac silhouette toward the sternum. This is also the operative side of many access routes, since venous cannulation for cardiopulmonary bypass often targets the superior and inferior venae cavae at their junctions with the right atrium, and device leads traverse this chamber en route to the right ventricle. Landmarks along the atrioventricular groove and the outflow tract help orient catheter paths and explain common sites of arrhythmia ablation around the tricuspid annulus and cavotricuspid isthmus.
