Superior Epigastric Vein, Anterior View

Description

Running vertically along the posterior surface of rectus abdominis, the superior epigastric vein is presented in anterior view as it descends from the thoracic wall into the anterior abdominal wall. Proximally, it relates to the internal thoracic (mammary) vein near the parasternal region, then courses inferiorly within the rectus sheath, medial to the semilunar line and deep to the rectus muscle fibers shown in the abdominal wall. Lateral thoracoabdominal musculature and the pectoralis major provide surface landmarks superiorly, while the midline linea alba anchors orientation. This anterior perspective matters because the superior epigastric vein sits at a crossroads between the internal thoracic venous system and the epigastric drainage of the anterior abdominal wall, making it a practical route for collateral flow when the inferior vena cava or iliac veins are obstructed. Prominence of superficial and paraumbilical veins is a classic bedside clue, but the deeper superior epigastric channel is the quieter partner you need to understand when mapping venous return across the thoracoabdominal junction. It is also a vessel at risk during paramedian incisions and subcostal approaches that enter the rectus sheath.
