Centered on the upper lip, the superior labial vein courses within the subcutaneous tissues of the philtrum and vermilion, running medial to the nasolabial fold before passing superolaterally to join the facial vein. Surrounding it, the orbicularis oris encircles the oral aperture, while the zygomaticus major and minor descend inferomedially from the zygomatic bone toward the modiolus, layering superficial to the buccinator. Superiorly, the alae and dorsum of the nose frame the venous pathway, and inferiorly the vein lies above the oral commissures and the lower border of the maxilla. Small. Easy to miss. Clinically, this anterior perspective clarifies why infections and injections of the upper lip demand respect: venous channels of the face are valveless and communicate with the angular vein at the medial canthus, creating a route toward the ophthalmic veins and cavernous sinus.
