Description
Arising at the superomedial orbit near the angular and supratrochlear veins, the superior ophthalmic vein courses posteriorly along the orbital roof, passing superior to the optic nerve as it tracks toward the orbital apex. In this superior section of the male skull, its blue venous channel can be followed as it converges on the superior orbital fissure, lying in close relation to the frontal bone anteriorly and the sphenoid body and lesser wing at the posterior orbit. The optic nerves run posteromedially from each globe toward the optic canals, while adjacent venous tributaries in the periorbital network drain into the dominant superior ophthalmic trunk. Bone is rendered in beige, arteries in red, and cranial nerves in a contrasting tone, clarifying the crowded neurovascular topography at the skull base. A superior perspective is the clearest way to appreciate why the superior ophthalmic vein matters clinically: it provides a principal venous outflow pathway from the orbit to the cavernous sinus via the superior orbital fissure, and it lacks valves, so infection and thrombus can propagate retrograde. This is the venous route implicated in septic cavernous sinus thrombosis from facial or sinus infection, and it is also the vessel radiologists often assess for dilation in carotid cavernous fistula, where arterialization produces a prominent, tortuous superior ophthalmic vein. Relationships at the orbital apex are tight. Millimeters count.
