Description
Arising as the terminal continuation of the inferior mesenteric artery, the superior rectal artery descends in the retroperitoneum and crosses the pelvic brim to reach the posterior aspect of the rectum. From a lateral perspective, its course is seen passing inferiorly from the sigmoid colon and mesocolon, then dividing into multiple descending branches that run along the rectal wall before penetrating the muscularis to supply the upper rectum. Adjacent sigmoid arteries and their accompanying veins form a recognizable vascular arcade along the sigmoid colon, while small pararectal and inferior mesenteric lymph nodes cluster along the vascular pedicle. Orientation is clear: the rectum lies inferior to the sigmoid colon, and the arterial branches track distally toward the anorectal junction. This view matters when you need to teach or plan surgery around the inferior mesenteric distribution and rectal perfusion. During low anterior resection or sigmoid colectomy, ligation level at the inferior mesenteric artery (high tie vs low tie) directly affects flow through the superior rectal artery and the marginal artery of Drummond, with implications for anastomotic ischemia and leak risk. It also helps frame the portosystemic anastomosis between the superior rectal vein (portal via inferior mesenteric vein) and middle and inferior rectal veins (systemic), a concrete anatomic basis for internal hemorrhoids and their bleeding pattern.
