Description
Running superficially along the posterolateral calf, the sural nerve is traced from its formation by contributions from the medial sural cutaneous nerve (tibial nerve) and the peroneal communicating branch of the common fibular nerve. It descends inferiorly adjacent to the small saphenous vein, then curves posterior to the lateral malleolus to continue distally along the lateral margin of the foot as the lateral dorsal cutaneous nerve. Deep to it lie the posterior compartment contours, with the gastrocnemius and soleus forming the bulk of the calf and the Achilles tendon narrowing toward its insertion on the calcaneus. Clear relationships. Clinically, the sural nerve is a frequent donor for nerve grafting and a common site of iatrogenic injury during lateral approaches to the ankle and hindfoot. Its close relationship to the small saphenous vein and the course behind the lateral malleolus explain why percutaneous procedures, endoscopic calcaneal work, or even aggressive varicose vein treatment can leave patients with dysesthesia along the lateral foot and fifth toe. This lateral perspective also aligns well with surface anatomy palpation and with the typical site for sural nerve biopsy in suspected vasculitic or amyloid neuropathy. Use this illustration for teaching peripheral nerve anatomy in gross anatomy labs, podiatric and orthopedic surgery modules, and anesthesia training when discussing sural nerve blocks or landmark-based injections around the lateral malleolus. It also fits cleanly into textbooks and patient-facing materials addressing lateral ankle surgery risks, Achilles procedures, and postoperative sensory mapping.
