Description
Seen from the lateral aspect of the male head and neck, the spinal accessory nerve (cranial nerve XI) descends deep to the sternocleidomastoid, then emerges at the posterior border of that muscle near the midpoint of the neck. From there it crosses the posterior triangle in an oblique inferior and posterior course toward the anterior border of the trapezius, accompanied superficially by cervical cutaneous branches from the cervical plexus (for example the lesser occipital and great auricular nerves) as they fan toward the auricle and lateral scalp. The parotidomasseteric region sits anterosuperiorly with masseter and temporalis defined, while the external ear provides a consistent surface landmark for orientation. For operative anatomy, this lateral perspective matters because the accessory nerve is most exposed where it traverses the posterior triangle, a common site of iatrogenic injury during lymph node biopsy or selective neck dissection (levels II to V). Injury produces trapezius weakness with shoulder droop, impaired abduction above horizontal due to loss of upward scapular rotation, and scapular winging that is lateral rather than medial as seen with long thoracic nerve palsy. Clean depiction of its relationship to sternocleidomastoid and the posterior triangle helps distinguish motor deficit from sensory symptoms carried by the cervical plexus in the same corridor.
