Description

Centered in the abdominal cavity, the jejunum and ileum form coiled loops bordered peripherally by the colon, creating the familiar frame of the large bowel around the small intestine. Proximally, the terminal ileum approaches the ileocecal junction at the cecum in the right lower quadrant, then the ascending colon courses superiorly to the hepatic flexure before turning medially as the transverse colon. Distally, the colon continues to the splenic flexure and descends along the left flank into the sigmoid colon, which curves inferomedially to become the rectum and anal canal. Color contrast separates small bowel from large bowel for rapid orientation. Colectomy refers to resection of a segment of colon with restoration of continuity (primary anastomosis) or diversion, depending on pathology and physiologic reserve. Understanding the segmental anatomy matters because vascular pedicles and lymphatic drainage track with each portion of the colon: right colectomy centers on the ileocolic and right colic territories, transverse colectomy on the middle colic, and left colectomy on the left colic and sigmoid branches, with the marginal artery forming a key collateral along the mesenteric border. It also clarifies where surgeons mobilize the bowel along embryologic fusion planes (white line of Toldt) and why splenic flexure mobilization can be required to achieve a tension-free anastomosis.
