Description

Arising as thin connective tissue bands at the level of the proximal interphalangeal joints, the transverse retinacular ligaments span transversely across each digit, linking the flexor tendon sheath and volar plate to the lateral margins of the extensor expansion. From a posterior (dorsal) hand view, the metacarpals and phalanges provide the osseous framework deep to the dorsal digital apparatus, while the extensor tendons course distally toward the middle and distal phalanges. The transverse fibers sit deep to the dorsal hood but act across the sides of the joint, tethering the lateral bands so they do not drift excessively dorsal or volar during finger motion. Subtle anatomy. Big consequences. Functionally, these ligaments help keep the lateral bands properly aligned around the axis of the PIP joint, so extension and flexion remain balanced between the central slip and the lateral bands. When the balance fails, classic deformities follow: volar migration of the lateral bands contributes to boutonniere deformity after central slip injury at the PIP, while dorsal migration can be part of the mechanism seen in swan-neck patterns and chronic imbalance after trauma or inflammatory arthropathy. This posterior perspective is also useful for teaching how apparently palmar stabilizers still influence the dorsal extensor mechanism through their attachments to the lateral bands and sheath.
