Running deep to rectus femoris, the vastus intermedius forms a broad, flattened muscle belly along the anterior and lateral surfaces of the femoral shaft, occupying the middle third of the thigh between the proximal femur and the suprapatellar region. Laterally, its fibers lie medial to vastus lateralis and anterior to the femur, with the muscle’s distal portion blending into the deep layer of the quadriceps tendon as it approaches the patella.
