Description
Deep to the superficial quadriceps layer, the vastus intermedius occupies the anterior and lateral surfaces of the femoral shaft, lying posterior to rectus femoris and between vastus lateralis laterally and vastus medialis medially. Proximally it arises from the proximal two thirds of the femur and adjacent intermuscular septa, then courses inferiorly to blend into the quadriceps tendon and insert, via the patella and patellar ligament, on the tibial tuberosity. Bony landmarks of the pelvis, femur, patella, tibia, and fibula frame the muscle’s position, while the sartorius is shown as a long strap muscle running from the anterior superior iliac spine obliquely to the medial proximal tibia, crossing the thigh superficially. Orientation is posterior with a slight medial bias, giving a clear sense of how the thigh musculature relates to the femoral diaphysis and knee joint line. Vastus intermedius is the workhorse extensor that often gets ignored because it sits deep, but it is the quadriceps component most directly tethered to the femur and is frequently involved in post-traumatic thigh pain after femoral shaft fracture, intramedullary nailing, or contusion. Its distal fibers contribute to the quadriceps tendon and patellar tracking mechanics, so weakness or inhibition can show up as extensor lag after knee arthroplasty or prolonged immobilization. Small, deep, and easy to miss. The juxtaposition with sartorius also helps teach the difference between a monoarticular knee extensor and a biarticular hip flexor and knee flexor that can mask true quadriceps deficit on manual testing.
