Description
Anterior lower-limb anatomy is rendered with the vastus intermedius exposed on the anterior surface of the femoral shaft, deep to the rectus femoris and bordered medially and laterally by vastus medialis and vastus lateralis within the quadriceps femoris group. Distally, its fibers converge toward the common quadriceps tendon inserting onto the base and margins of the patella, continuing as the patellar ligament to the tibial tuberosity. The femur, patella, tibia, fibula, and tarsal and metatarsal bones remain in clear relief, with the ankle and foot tendons extending distally in the anterior compartment. Orientation is anterior, with lateral structures subtly favored. Deep placement makes the vastus intermedius easy to overlook, yet it is the quadriceps component most directly apposed to the femur and a key contributor to knee extension across mid-range motion. This relationship matters in anterior thigh approaches and femoral shaft surgery, where dissection through or between quadriceps planes can affect postoperative knee strength and scarring, and in quadriceps tendon injuries where fiber contributions to the suprapatellar region are considered. A clean landmark. Patellar alignment and patellofemoral pain discussions often benefit from clarifying how quadriceps forces converge onto the patella before transmitting through the patellar tendon.
