Description
Prominently along the anteromedial thigh, the vastus medialis is rendered as the medial component of the quadriceps femoris, with its distal oblique fibers thickening into the characteristic teardrop contour just superior and medial to the patella. Laterally, the vastus lateralis and iliotibial tract define the outer thigh, while the femoral shaft lies deep to the quadriceps muscle bellies. Distal to the knee, the patella, tibial tuberosity, and patellar ligament align on the anterior midline, with the tibia medial and the fibula lateral; the Achilles tendon anchors posteriorly at the calcaneus. Orientation is clear. Clinically, the distal vastus medialis, often discussed as the vastus medialis obliquus (VMO) region, matters because its fiber direction contributes to medial control of patellar tracking during knee extension. When this medial pull is weak or delayed relative to the vastus lateralis, patients can present with lateral patellar tilt, anterior knee pain, and recurrent patellofemoral symptoms, a pattern frequently addressed in rehabilitation and sports medicine. It also provides a practical surface landmark for locating the superomedial border of the patella during examination and for explaining why medial quadriceps atrophy becomes visible early after knee injury or surgery.
