Description
Anterior thigh anatomy centers on the vastus intermedius, a broad quadriceps femoris component that lies deep to rectus femoris and conforms tightly to the anterior and lateral surfaces of the femoral shaft. Medially it approaches vastus medialis, laterally it blends toward vastus lateralis, and its fibers run inferiorly to converge into the deep portion of the quadriceps tendon. Superiorly, the muscle arises from the proximal two thirds of the femur (including the linea aspera region), while distally it contributes to the suprapatellar region just proximal to the patella. Deep and flat. Appreciating this anterior relationship matters when teaching layered dissection of the quadriceps, because the vastus intermedius is often obscured until rectus femoris is reflected and the anterior femur becomes the reference surface. In anterior knee pain and post operative extensor lag, weakness or inhibition of the deep quadriceps can be missed if attention stays on rectus femoris alone, and MRI commonly shows edema within vastus intermedius after direct contusion to the mid thigh. Surgeons also encounter the muscle during anterior and anterolateral approaches to the femur for intramedullary nailing or plating, where splitting the quadriceps and respecting its femoral nerve supply reduces postoperative stiffness and scarring.
