Description
Oriented from a posterior approach, the right and left kidneys sit retroperitoneally on either side of the vertebral column, their hila directed anteromedially toward the great vessels. Blue renal veins emerge from each hilum and course medially to join the inferior vena cava on the right, while the left renal vein crosses the midline anterior to the aorta to reach the cava. Yellow neural elements trace along the posterior abdominal wall near the renal pedicles, and green lymphatic channels and paraaortic nodes track the same longitudinal corridor. Depth cues matter. Renal venous anatomy is where laterality stops being symmetric, and this posterior view makes that asymmetry easy to teach. The longer left renal vein and its relationship to the aorta and superior mesenteric artery underlie nutcracker syndrome, a cause of hematuria, flank pain, and pelvic congestion, and it is also the vein most often mobilized during left nephrectomy or donor nephrectomy. Posterior exposure also mirrors the operative field for retroperitoneal approaches, where the inferior vena cava and renal veins define the safe plane before hilar clamping.
