Within the renal parenchyma, interlobar veins run between the renal pyramids toward the corticomedullary junction, where arcuate veins sweep along the base of the pyramids before giving off smaller cortical radiate venules that drain the cortex. Red renal arteries track close to their venous counterparts, and the ureters descend inferiorly from the renal pelvis, posterior to the peritoneal plane, with perinephric fat framing the organ. Venous anatomy matters in the kidney because the low pressure drainage pathways dictate both imaging appearances and surgical risk, and small variations at the hilum can turn a clean dissection into uncontrolled bleeding. Nephrectomy, partial nephrectomy, and donor procurement all hinge on recognizing the main renal vein and its segmental tributaries before dividing hilar structures, especially on the right where the renal vein is shorter and reaches the inferior vena cava quickly. This view also supports teaching of venous return from the cortex, through arcuate and interlobar channels, into the renal vein, a pathway often referenced when interpreting renal congestion, venous thrombosis, or collateral formation in caval obstruction.
