Description
Centered on the posterior spine, the external vertebral venous plexus is rendered as a bilateral network coursing along the laminae and spinous processes from the cervical region through the thoracolumbar segments. Interconnecting channels run transversely across adjacent bony plates and communicate laterally with the intervertebral veins at the level of the intervertebral foramina, where segmental veins continue toward the azygos system and lumbar veins. Deep to the posterior elements, the vertebral canal is suggested through transparency, with the spinal cord aligned centrally and paired spinal nerves exiting laterally in a metameric pattern. No valves. Because these veins are valveless and interconnect freely with the internal vertebral (epidural) venous plexus, this posterior emphasis matters when you teach or explain pathways of hematogenous spread that bypass the caval system. Increased intraabdominal or intrathoracic pressure can reverse flow through the vertebral venous plexuses, a mechanism often cited in vertebral metastases from pelvic malignancy and in epidural venous engorgement contributing to radicular symptoms. The posterior perspective also supports surgical orientation during midline posterior approaches, where venous bleeding can arise from plexiform channels near the posterior elements and foraminal region. Common use cases include neurosurgery and orthopedic spine teaching on epidural space anatomy, anesthesiology discussions of unintended intravascular uptake during epidural procedures, and oncology or pathology figures addressing Batson plexus related metastatic patterns.
