Description

Running along the posterior elements of the cervical vertebrae, the external vertebral venous plexus forms a longitudinal, valveless network superficial to the laminae and spinous processes, with interconnections that track segmentally across the intervertebral spaces. Small collecting veins communicate medially with the internal vertebral venous plexus in the epidural space through the intervertebral foramina, adjacent to the exiting cervical spinal nerves. Laterally, these channels align with the transverse processes and expected drainage pathways toward the deep cervical and vertebral veins. Spatially, the plexus sits dorsal to the vertebral bodies and intervertebral discs, while the nerve roots course anterolaterally through the foramina. Valveless flow is the point. Because the vertebral venous plexuses lack valves, pressure shifts from coughing, Valsalva, or abdominal compression can reverse flow and transmit infection or metastases along the spine, a mechanism often cited in spread from pelvic organs to vertebrae and the cranial cavity. In the cervical region, these communications also matter during posterior approaches and instrumented fusion, where brisk venous bleeding can occur from the epidural and paravertebral venous channels near the lamina and foramen. The relationship to the foramina is a practical landmark.
