Description
Coronal section through a tooth reveals the enamel cap externally, dentin deep to it, and a central pulp chamber that tapers inferiorly into one or more root canals. Within the pulp space, neurovascular bundles (pulpal nerves with accompanying arterioles and venules) course longitudinally from the apical foramen toward the coronal pulp, then arborize into smaller branches near the pulp horns. Connective tissue sheaths surround these bundles, with the microvasculature running alongside nerve fascicles within the endoneurial compartment and collectively wrapped by perineurium, then epineurium as the bundle enlarges. Color coding distinguishes arteries from veins and tracks neural pathways through the root. Dental pain lives here. In pulpitis, inflammatory edema develops inside a rigid dentin shell, raising intrapulpal pressure and compressing venules and nociceptive fibers, which explains the classic throbbing, heat-sensitive toothache and the rapid progression toward ischemic necrosis when drainage is limited. The apical third and apical constriction also matter during endodontic instrumentation, since overextension can injure the apical neurovascular supply, extrude irrigants, and worsen post-treatment flare-ups or paresthesia in adjacent sensory territories.
