Description
Posterior anatomy of the male urinary tract is rendered with both kidneys positioned superiorly and retroperitoneally, their renal pelves tapering into the proximal ureters that descend inferomedially toward the urinary bladder. Fine ureteric nerve filaments are traced along the periureteric sheath, closely paralleling segmental vessels and forming a periureteric plexus that remains posterolateral to the ureter for much of its course. Inferiorly, the bladder sits in the midline, and the prostate occupies the immediately caudal position at the bladder base, consistent with male pelvic anatomy. Orientation is clean. This posterior perspective matters because ureteral autonomic innervation is encountered, stretched, or thermally injured during retroperitoneal and pelvic surgery, including ureterolysis for retroperitoneal fibrosis, lymph node dissection, and mobilization of the distal ureter during prostatectomy or cystectomy. The view also supports teaching of referred pain pathways: ureteral distension from calculus can produce flank-to-groin pain via sympathetic afferents that track with the ureteric plexus and enter spinal segments commonly cited as T11 to L2.
