Description

Anterior orientation places both kidneys superiorly and lateral to the vertebral column, with the renal pelvis narrowing into the ureters as they descend on the posterior abdominal wall toward the urinary bladder in the midline of the pelvis. Fine ureteric nerve filaments are shown accompanying the ureter along its course, tracking in close apposition to the adventitia rather than entering the lumen. Inferiorly, the urethra exits the bladder neck and continues caudally as the male urethra. Small nerves, big consequences. Clinically, this relationship matters during ureteric mobilization and distal ureter dissection in men, where traction, cautery spread, or ligation can interrupt autonomic input that coordinates ureteral peristalsis and ureterovesical junction function. The anterior view also clarifies where the ureter approaches the bladder wall, a common site for iatrogenic injury during pelvic surgery and a frequent location for obstructing calculi, with resultant hydroureter and hydronephrosis proximal to the blockage.
