Description
Seen in lateral profile, the kidney sits superiorly with the renal pelvis funneling into the ureter, which descends retroperitoneally toward the urinary bladder in the lesser pelvis. A fine ureteric nerve plexus tracks along the ureter’s adventitia, forming a longitudinal network that remains closely apposed to the ureter from its proximal segment near the ureteropelvic junction to its distal intramural course at the bladder wall. Posteriorly, the ureter lies against the psoas major region in the abdominal segment, then continues inferiorly toward the posterolateral aspect of the bladder. This relationship matters because ureteral peristalsis is driven by smooth muscle and modulated by autonomic input traveling through the renal, aortic, and hypogastric plexuses, with visceral afferents returning to spinal segments that explain classic flank-to-groin pain referral in ureterolithiasis. A lateral view makes the retroperitoneal pathway easier to teach, including the predictable sites of obstruction at the ureteropelvic junction, pelvic brim, and ureterovesical junction. Small fibers are easy to overlook. They are not optional in pelvic surgery.
