Description
Positioned superior to the upper pole of the kidney, the suprarenal (adrenal) gland is rendered in cross section to distinguish cortex from medulla while preserving its anatomic relationship to the renal capsule and the concavity toward the medial border. A thin fibrous capsule outlines the gland, deep to which the adrenal cortex forms a broad peripheral band organized into zona glomerulosa (most superficial), zona fasciculata (thickest middle layer), and zona reticularis (inner cortical layer) surrounding a central adrenal medulla. Medially, the adrenal sits closer to the renal hilum region, while the kidney’s smooth external surface and hilar indentation orient the specimen in a sagittal-style sectional context. Layer recognition matters here because cortical and medullary disease behaves differently and is approached differently in imaging, pathology, and surgery. Cortisol-producing adenomas and hyperplasia arise from the cortex, while pheochromocytoma originates in chromaffin cells of the medulla and carries perioperative risk from catecholamine release; the ability to point to the medulla relative to the cortical zones helps explain why biochemical workups and contrast-enhanced CT or MRI protocols target different lesion characteristics. Surgeons also care about this topography during adrenalectomy, where the short central adrenal vein (right directly to the IVC, left typically to the left renal vein) and the gland’s superior relation to the kidney drive exposure and vascular control.
