Description
Prominent within the orbital cavity, the ocular globe is shown in section with the sclera forming the outer coat, the cornea and anterior chamber anteriorly, and the iris encircling the pupil. Arterial channels (red) course from the posterior orbit toward the globe around the optic nerve, then divide into branches that run circumferentially near the equator and extend anteriorly toward the ciliary body and posteriorly toward the choroid and retina. Venous pathways (blue) accompany the arterial supply but converge posteriorly toward the orbital apex, where drainage narrows into larger caliber vessels. Surrounding orbital bones frame the contents, emphasizing how tightly the vascular network occupies the conical space behind the eye. Orbital vascular anatomy matters because small positional differences at the apex can translate into large clinical consequences, from compressive optic neuropathy in orbital apex syndrome to hemorrhage in retrobulbar hematoma after trauma or periocular anesthesia. The relationship of the ophthalmic artery and its ocular branches to the optic nerve is a common teaching stumbling block, and this sectional presentation clarifies why elevated intraorbital pressure can compromise both retinal perfusion and venous outflow.
