Description

Positioned within a sectioned male skull, the globe (bulbus oculi) sits in the orbital cavity with the cornea and blue iris anterior and the posterior pole directed toward the orbital apex. A stout optic nerve (cranial nerve II) exits the posterior aspect of the eyeball and courses posteriorly toward the optic canal, while finer yellow filaments cluster around the posterior sclera and converge near the annulus of Zinn. Bony orbital margins and apertures frame the soft tissues, and superficial episcleral vessels trace across the sclera. Clinical relevance concentrates at the orbital apex, where the optic nerve, ophthalmic artery, and the oculomotor, trochlear, and abducens nerves enter through the optic canal and superior orbital fissure in close quarters. Small changes in volume here, from an orbital apex tumor, thyroid eye disease, or an apical hematoma after trauma, can produce early optic neuropathy with painful ophthalmoplegia, the classic orbital apex syndrome pattern that learners often struggle to map in three dimensions. This is tight anatomy. It also supports surgical orientation for medial orbital decompression and for approaches that must respect the optic nerve’s relationship to the posterior pole and extraocular muscle cone.
