Description

Arising from the common tendinous ring (annulus of Zinn) at the orbital apex, the superior, inferior, medial, and lateral rectus muscles course anteriorly along the orbital walls to insert on the sclera anterior to the equator, forming a muscular cone around the optic nerve. The superior oblique originates superomedially and runs anteriorly toward the trochlea before turning posterolaterally to the globe, while the inferior oblique begins on the anteromedial orbital floor and sweeps posterolaterally beneath the eyeball. Cranial nerve relationships are emphasized: the oculomotor nerve (CN III) supplies the superior, inferior, and medial recti plus inferior oblique, and the abducens nerve (CN VI) enters the cone to innervate the lateral rectus. Small episcleral vessels trace over the sclera near the limbus. The iris and pupil are shown anteriorly in situ. For teaching ocular motility, the shared origin of the recti at the orbital apex is the anchor point, it explains why orbital apex lesions can produce combined ophthalmoplegia with optic neuropathy. This anatomy also underpins strabismus patterns: a CN VI palsy weakens abduction via the lateral rectus, while an isolated CN III palsy affects adduction, elevation, depression, and often produces ptosis and a down-and-out resting position from unopposed lateral rectus and superior oblique. Surgeons reference these insertions during extraocular muscle recession and resection, and anesthetists consider the intraconal space when planning retrobulbar blocks.
