Description
Wrapped around the scleral globe, the superior, inferior, medial, and lateral rectus muscles course from the posterior orbit toward their anterior scleral insertions, forming a muscular cone around the optic nerve. Superior and inferior oblique muscles arc to the posterolateral sclera, with the superior oblique tendon angling anterosuperomedially toward the trochlear region and the inferior oblique sweeping laterally from the anteromedial orbital floor. An anterior segment of the eye is visible with cornea and iris, while the optic nerve exits posteriorly. Fine episcleral vessels track across the sclera. Extraocular muscle anatomy becomes clinically concrete when you correlate action with innervation in the tight orbital space: the oculomotor nerve (CN III) supplies most of the recti and the inferior oblique, the trochlear nerve (CN IV) targets superior oblique, and the abducens nerve (CN VI) drives lateral rectus, so a single palsy yields a predictable motility deficit and diplopia pattern. This view makes it easier to teach why a CN VI palsy produces impaired abduction and an esotropic resting position, while thyroid eye disease preferentially restricts inferior rectus excursion and alters elevation in abduction. Small distances matter. A millimeter-scale shift in insertion or tendon course changes the vector.
