Description
Running from the transverse processes of C1 to C4 down to the superior angle and adjacent medial border of the scapula, the levator scapulae is presented bilaterally as a firm, strap-like muscle on the posterolateral neck. Its fibers descend inferiorly and laterally, lying deep to the upper trapezius and posterior to the sternocleidomastoid region, with the cervical spinous processes and nuchal contour providing midline orientation. The scapulae sit lateral to the thoracic spine, and the levator scapulae inserts at the superomedial scapular corner just superior to the rhomboid minor attachment. Clinically, this view helps clarify why levator scapulae pain so often feels like a neck problem but behaves like a scapular problem: the muscle couples cervical side-bending and rotation with scapular elevation and downward rotation, so restricted scapulothoracic motion can perpetuate cervical overload. Trigger points near the superior angle of the scapula commonly refer pain along the posterolateral neck and toward the medial scapular border, a frequent pattern in desk-related neck pain and after whiplash.
