Description
Positioned on the upper posterior trunk of an adult male, the levator scapulae runs obliquely from the posterior tubercles of the transverse processes of C1 to C4 to the superior angle and adjacent superior part of the medial border of the scapula. Its cervical attachments sit lateral to the spinous processes and deep to the more superficial trapezius, while its scapular insertion lies superior to the rhomboid minor along the vertebral (medial) border. Bilateral scapulae, upper ribs, the occipital bone, and the cervical to upper thoracic vertebrae frame the muscle’s course in anatomic position. Small but clinically loud. Because the levator scapulae couples cervical side-bending with scapular elevation and downward rotation, it is a frequent generator of cervicoscapular pain that clinicians map to the superomedial scapular corner and posterior neck. Spasm or shortening contributes to postural syndromes with elevated scapula and restricted cervical rotation, and it can mimic or coexist with cervical radiculopathy, especially when symptoms localize near the C3 to C5 dermatomes. The dorsal scapular nerve (C5), often coursing deep to levator scapulae before reaching the rhomboids, also makes this region relevant during posterior triangle dissection, ultrasound-guided injections, and trigger point therapies where accurate depth relationships matter.
