Description
Running just deep to the cervical skin, the platysma forms a thin, sheet-like muscle within the superficial fascia of the anterolateral neck. Its superior fibers sweep toward the inferior border of the mandible and lower face, while inferior fibers descend over the clavicle to blend with fascia over the upper thorax. Lateral to it, the sternocleidomastoid muscle stands out as a paired strap from mastoid process to sternum and clavicle, framing the anterior cervical region and separating anterior from posterior triangles. Portions of trapezius across the shoulder girdle and the superior part of pectoralis major on the chest provide additional landmarks for orientation to the neck-shoulder junction. This superficial layer matters clinically because it is the first muscular plane encountered in anterior neck exposure and in facial plastic procedures that manipulate the subcutaneous tissues. The platysma is commonly tightened or divided in rhytidectomy and neck lift surgery, and it can form anterior midline bands with aging that guide surgical planning and injection patterns. Injury to the cervical branch of the facial nerve during subplatysmal dissection leads to lower lip depressor weakness, and the superficial position of the platysma makes it a practical reference when discussing safe planes relative to the external jugular vein and superficial cervical fascia.
