Description
Spanning the anterior neck as a thin subcutaneous sheet, the platysma lies immediately deep to the skin and superficial cervical fascia, extending inferiorly over the upper thorax toward the clavicle and superiorly to the inferior border of the mandible and lower face. Medially, right and left platysma fibers approach the midline beneath the submental region, while laterally the muscle thins toward the shoulder and blends near the superficial fascia over the deltopectoral area. Deeper, the sternocleidomastoid contours are readable as paired oblique columns flanking the anterior cervical midline, with portions of the infrahyoid strap muscles suggested centrally beneath the platysmal sheet. Color separation emphasizes the superficial-to-deep relationship across the cervical region and upper chest. Appreciating this anterior perspective matters because the platysma defines surgical planes and surface landmarks for the anterior neck, where a few millimeters separate subdermal dissection from the superficial cervical fascia and the investing layer of deep cervical fascia overlying the sternocleidomastoid. Platysmal decussation and midline separation help explain anterior neck banding, a common aesthetic and age-related finding addressed with platysmaplasty or neuromodulator injection, and also influence the approach during submental liposuction. The cervical branch of the facial nerve courses in this neighborhood as it innervates platysma, so an accurate sense of depth and fiber direction reduces the risk of iatrogenic lower lip asymmetry when working near the inferior mandible. Use this illustration in head and neck anatomy labs to teach superficial cervical fascia, cervical surface anatomy, and the relationship of platysma to sternocleidomastoid and the infrahyoid compartment, and in operative atlases covering rhytidectomy, submental approaches, and anterior neck dissection.
