Description
Seen in lateral profile, the platysma forms a thin, superficial muscular sheet draped over the anterolateral cervical region of an adult male, extending superiorly to the inferior border of the mandible and inferiorly toward the upper thorax and shoulder girdle. Its fibers run obliquely across the superficial cervical fascia, lying superficial to the sternocleidomastoid and the deeper infrahyoid compartment, and blending superiorly with perioral soft tissues near the angle of the mouth. Anterior to the ear and parotid region, the muscle approaches the lower face while remaining separated from deeper neurovascular structures by the investing layer of deep cervical fascia. Superficial anatomy, clearly mapped. For head and neck teaching, this lateral view clarifies why the platysma behaves as both a cervical muscle and a muscle of facial expression, depressing the lower lip and drawing the corner of the mouth inferolaterally while tensing the skin of the neck. It also frames the operative plane used in rhytidectomy and neck lift procedures, where platysmal banding and midline decussation (or lack of it) guide decisions about anterior platysmaplasty, lateral tightening, or chemodenervation with botulinum toxin. Understanding its relationship to the mandibular margin is practical when discussing the course of the marginal mandibular branch of the facial nerve, which is at risk during submandibular and lower facial incisions.
