Description
Prominent within the male retroperitoneum, the abdominal aorta descends anterior to the vertebral bodies and gives rise to the right and left renal arteries at approximately the L1 to L2 level, with each vessel coursing laterally toward its respective renal hilum. The right renal artery typically passes posterior to the inferior vena cava before reaching the kidney, while the left renal artery takes a shorter, more direct path from the aorta. Near the renal hilum, each artery divides into prehilar segmental branches that will supply discrete renal segments, arranged around the renal pelvis and calyces. Bony landmarks including the lower thoracic ribs, thoracolumbar spine, and superior pelvis frame these vascular relationships in anterior projection. Prehilar branching matters because segmental renal arteries behave as end arteries, so their occlusion or ligation can create sharply demarcated segmental renal infarction. Surgeons rely on this anatomy during nephron-sparing surgery and renal hilar dissection, where controlled clamping of a segmental artery can limit ischemia to a targeted parenchymal territory, and where unexpected accessory renal arteries can complicate hemostasis. Orientation to the aorta and inferior vena cava is the point. It also aligns with angiographic interpretation in renovascular hypertension workups, including atherosclerotic renal artery stenosis and fibromuscular dysplasia patterns.
