Cut away through the anterior segment and vitreous body, the globe opens onto the neurosensory retina lining the posterior wall of the eye, with the optic disc (papilla nervi optici) marking the point where the central retinal artery and central retinal vein enter and exit within the optic nerve head. The translucent lens sits posterior to the pupil and anterior to the vitreous, suspended by zonular fibers that span from the ciliary processes to the lens equator. Circumferentially, the ciliary body forms a muscular ring just posterior to the iris, while the sclera and underlying choroid provide the outer coats around the retinal layer. Posterior is where the retina matters. Clinically, the relationship between the optic disc and the central retinal vessels is the framework for understanding papilledema, optic neuritis, and glaucomatous cupping, all conditions where disc contour and peripapillary retina change in characteristic ways. The ciliary muscle and zonule-lens complex shown anteriorly anchors accommodation physiology and helps explain zonular dialysis after blunt trauma and the mechanism of lens subluxation in pseudoexfoliation syndrome. This combination of posterior fundus landmarks with anterior segment mechanics supports teaching that ties ophthalmoscopic findings to real anatomy rather than a flat diagram.
