Description
Centered on the posterior upper torso, the intermediate layer of deep fascia is rendered as an aponeurotic sheath spanning the midline from the external occipital protuberance and nuchal region inferiorly over the thoracic spinous processes. Laterally, the fascial plane blends toward the acromial region and scapular spine, investing the trapezius superficially while separating it from the rhomboid major and minor along the medial border of the scapula. Inferior and lateral extensions relate to the superficial surface of latissimus dorsi, with the fascia tightening into broad tendinous expansions near the midline. Clear planes. This posterior perspective matters because the deep fascia and its intermediate lamina define the surgical and injection planes used around the scapulothoracic region, where small shifts in depth can place a needle in trapezius instead of the rhomboid layer or along the thoracic paraspinal compartment. Clinically, that distinction comes up in ultrasound guided rhomboid intercostal plane blocks and scapular border approaches for myofascial pain, and in operative dissection where the trapezius is elevated off its investing fascia to reach the dorsal scapular nerve and vessels deep to the rhomboids. The aponeurotic continuity also helps explain how tension transmits across the posterior shoulder girdle during scapular retraction and depression.
