Description
Centered over the lumbar spinous processes, the thoracolumbar fascia forms a broad aponeurotic sheet that spans inferiorly toward the posterior iliac crest and laterally toward the flanks, where it blends with the aponeuroses of the lateral abdominal wall. The superficial (posterior) lamina lies posterior to the erector spinae muscle mass, while deeper laminae invest the paraspinal compartment and relate anteriorly to quadratus lumborum and the transverse processes of the lumbar vertebrae. Laterally, fibers interdigitate with the latissimus dorsi, and inferiorly the fascia approaches the sacrum and superior gluteal region. Bony landmarks anchor the field. Posterior anatomy of the thoracolumbar fascia matters because this sheath is not just packaging, it is the main load-sharing interface between trunk extensors and the pelvis, and it defines the fascial planes encountered in posterior lumbar approaches. Surgeons crossing the midline during posterior spine exposure incise the posterior lamina to mobilize the paraspinal muscles from the spinous processes, and the integrity of this layer influences postoperative pain and seroma formation. It also frames common pain generators: altered tensioning of the thoracolumbar fascia has been implicated in mechanical low back pain, and it is a key landmark when teaching how latissimus dorsi and gluteus maximus transmit force through the lumbopelvic junction.
