Description
Seen in left lateral profile, the nuchal fascia forms a fibrous sheet over the posterior cervical region, blending medially with the ligamentum nuchae along the midline and extending laterally over the upper trapezius toward its attachments on the occipital region. Anterior to this fascial plane, the sternocleidomastoid runs inferoanteriorly from the mastoid process toward the manubrium and clavicle, while the masseter and temporalis occupy the lateral face and temporal fossa superiorly. The auricle sits lateral to the mastoid region, providing a fixed surface landmark, and deeper contours suggest the occipital bone and upper cervical vertebrae beneath the muscle layers. A lateral view like this clarifies the relationship between the nuchal aponeurosis, the ligamentum nuchae, and the superficial muscle envelope of the posterior neck, which is where dissection often goes wrong when layers are lifted en bloc. That matters in posterior cervical approaches, including midline exposure for C1 to C2 instrumentation, where the surgeon works through trapezius and the midline raphe to stay on the ligamentum nuchae and limit bleeding and denervation. It is also a practical teaching frame for myofascial pain patterns at the occipital attachment of trapezius and for the fascial continuity that can transmit tension into cervicogenic headache complaints.
