Description
Seen in lateral profile, the pretracheal fascia forms a cervical sheath deep to the platysma and investing layer of deep cervical fascia, spanning from the hyoid region inferiorly toward the superior mediastinum. It encloses the infrahyoid strap muscles anterior to the larynx and trachea and splits to invest the thyroid gland, hugging the gland’s anterolateral surface as it sits anteroinferior to the thyroid cartilage and cricoid cartilage. Posterior to this fascial compartment lie the pharynx and cervical esophagus, while the sternocleidomastoid and shoulder girdle musculature provide lateral surface context for depth and layering. Cartilaginous landmarks of the larynx appear in blue, helping orient the fascia around the airway. For teaching and operative planning, this specific fascial plane matters because it explains how infection, hemorrhage, and air track in the anterior neck and into the chest. The pretracheal space can communicate with the anterior mediastinum, a pathway relevant in descending necrotizing mediastinitis after odontogenic or deep neck infection, and it is also encountered during tracheostomy and thyroidectomy when surgeons divide midline cervical fascia to reach the trachea and thyroid isthmus.
