Description
Posterior trunk anatomy is presented with emphasis on the deep fascia (thoracolumbar fascia) as a dense aponeurotic sheath spanning the midline over the spinous processes and investing the erector spinae (sacrospinalis) mass. Superficially, the trapezius occupies the superior back from the occipital region toward the acromion and scapular spine, while the latissimus dorsi spreads inferolaterally from the lower thoracic and lumbar region toward the posterior axillary fold. Laterally, the fascia blends with the abdominal wall aponeuroses and the iliac crest, while superiorly it continues toward the nuchal fascia and deep cervical fascia. Planes matter here. Thoracolumbar fascia is the key partition between superficial back musculature and the paraspinal compartment, and it behaves as more than wrapping: it transmits load between the pelvis, spine, and upper limb through attachments to the lumbar transverse processes, sacrum, and iliac crest, with functional continuity into the latissimus dorsi and contralateral gluteus maximus via the posterior layer. That linkage becomes clinically concrete in posterior approaches to the lumbar spine, where splitting the thoracolumbar fascia and separating multifidus from longissimus along the natural cleavage plane can reduce muscle denervation and postoperative pain. It also frames why paraspinal compartment syndrome, though uncommon, produces firm swelling and pain with extension, the fascia limits expansion.
