Viewed from posterior, the serous pericardium is presented as the parietal layer lining the fibrous pericardium, draping over the diaphragmatic surface of the heart and reflecting onto the great vessels to become visceral pericardium (epicardium). The posterior left atrium sits centrally, with the pulmonary veins entering its superior and lateral margins, while the inferior vena cava approaches the right atrium inferiorly. Coronary vessels course within epicardial fat along the atrioventricular groove and posterior interventricular sulcus, with venous channels tracking toward the coronary sinus on the posterior aspect. The pericardial reflections form sleeves around the aorta and pulmonary trunk superiorly and around the venae cavae and pulmonary veins posteriorly. Clear landmarks. A posterior perspective on the serous pericardium matters because the oblique pericardial sinus lies between the posterior left atrium and the pericardial reflections around the pulmonary veins, a space surgeons use to mobilize the heart and pass a clamp or tape during cardiopulmonary bypass and left atrial procedures.
