Description
Anteriorly, the fibrous pericardium forms a tough outer sac around the cardiac silhouette, with the parietal layer of serous pericardium lining its deep surface and reflecting onto the visceral pericardium (epicardium) over the ventricles and atria. Superiorly, the pericardial sac blends with the adventitia of the great vessels, so the ascending aorta and pulmonary trunk emerge from within the pericardial reflection, while the superior vena cava enters on the right. Along the anterior surface, the right ventricle sits most anterior and the left ventricle lies inferolateral to it, with coronary vessels coursing in the interventricular and atrioventricular grooves beneath the glossy epicardium. Subtle transparency hints at deeper chamber geometry without breaking the external serosal relationships. A thin interface, but a decisive one. This anterior perspective is the one clinicians think in when discussing pericardial disease, because pericardial thickening, adhesions, or effusion separate the fibrous sac from the epicardial surface and change how the heart fills.
