Description
Anteriorly, the liver occupies the right hypochondrium and epigastrium beneath the diaphragm, with the smooth visceral peritoneum (hepatic serosa) reflecting over its convex surface and demarcating the nonperitoneal bare area superiorly. The right lobe lies lateral and superior to the left lobe, separated on the diaphragmatic surface by the falciform ligament line, while the inferior margin of the liver overhangs the stomach and proximal duodenum. Inferior to the hepatic border, the stomach sits mainly to the left of midline, and loops of small intestine fill the central abdomen, framed peripherally by the large intestine. Serosal anatomy matters because peritoneal reflections define surgical planes and the routes by which fluid, blood, and malignant cells track within the peritoneal cavity. On the liver, visceral peritoneum covers most of the organ but leaves the bare area in direct contact with the diaphragm, a relationship that explains why subphrenic collections can be difficult to drain and why hepatic trauma may dissect along peritoneal and subperitoneal planes. The anterior perspective also helps teach the distinction between parietal peritoneum lining the abdominal wall and visceral peritoneum investing the liver, a point that underpins interpretation of peritoneal signs and postoperative adhesions.
