Seen from posterior, the visceral peritoneum (serosa) invests the posterior and superior hepatic surface and reflects onto adjacent structures, outlining the bare area of the liver where peritoneum is absent. Superiorly, peritoneal reflections converge toward the diaphragm around the coronary ligament, with right and left triangular ligaments forming at the lateral extremes, and the visceral layer turning inferiorly toward the lesser omentum at the porta hepatis. Medial and posterior contours imply the relationship of the caudate lobe and fissural region to the inferior vena cava groove, while the stomach and distal esophagus lie anteromedial to the left lobe. This posterior perspective matters because it is where surgeons and interventionalists mentally place the peritoneal boundaries that separate hepatic capsule, bare area, and subphrenic and subhepatic spaces. Fluid from perforated peptic ulcer, pancreatitis, or postoperative bile leak often tracks into the lesser sac and right subhepatic recess (Morison pouch), and the peritoneal folds around the porta hepatis frame the route of the hepatoduodenal ligament and Pringle maneuver. The coronary ligament and bare area also anchor the liver to the diaphragm, a practical concern during mobilization for right hepatectomy or transplantation.
