Description

Seen from posterior, the parietal peritoneum lines the posterior abdominal wall and reflects onto viscera as visceral peritoneum, forming the serosal envelope around the stomach, liver, small intestine, and large intestine. Mesenteric attachment lines are readable as oblique root of the mesentery for jejunum and ileum crossing from the left upper quadrant toward the right iliac fossa, with adjacent peritoneal reflections around the duodenum and pancreas. Retroperitoneal kidneys sit posterior to the peritoneal cavity on either side of the vertebral column, capped superiorly by the suprarenal (adrenal) glands, while the colon frames the central small-bowel loops more anteriorly. A posterior perspective is the clearest way to teach what is intraperitoneal versus retroperitoneal, and where peritoneal reflections tether bowel to the posterior wall. Surgeons and radiologists both rely on these attachment planes when tracking the spread of fluid, pus, or blood, for example along mesenteric and paracolic pathways in perforated appendicitis or diverticulitis, or when explaining why the pancreas and most of the duodenum behave as secondarily retroperitoneal organs. It is also the orientation that makes the root of the mesentery a practical landmark for small-bowel volvulus and internal hernia patterns.
