Description
Anteriorly, the fibrous pericardium and parietal serous pericardium form a pale outer sac around the centrally placed heart, while the visceral layer (epicardium) closely invests the cardiac surface and reflects onto the roots of the great vessels. Superiorly, the pericardial reflections surround the ascending aorta, pulmonary trunk, and the superior vena cava, creating the transverse and oblique pericardial sinuses at the base of the heart. The pericardium sits posterior to the sternum and costal cartilages, with the lungs and mediastinal pleura positioned laterally; the trachea and main bronchi appear superior and posterior to the pericardial sac. Clinically, an anterior perspective clarifies why pericardial effusions collect in the pericardial cavity between parietal and visceral layers, and how rising intrapericardial pressure can precipitate cardiac tamponade by restricting diastolic filling. It also supports procedural planning for pericardiocentesis, where needle trajectory is chosen to avoid the internal thoracic vessels and pleura, commonly via a subxiphoid approach aimed toward the left shoulder or a parasternal approach through the left 5th or 6th intercostal space. Pericarditis and postsurgical adhesions make these pericardial planes matter.
