Description
Posterior cardiac anatomy comes into focus at the base of the heart, where the left atrium forms most of the diaphragmatic and posterior surface and receives the pulmonary veins on its right and left sides. Inferior to the atrial base, the left ventricle contributes to the left portion of the diaphragmatic surface, while the right atrium lies more to the right, blending into the venous inflow tract. The descending thoracic aorta runs posterior and left of the atrium, and the pulmonary trunk with its right and left pulmonary arteries sits superiorly, with the transverse sinus region implied between the arterial and venous poles. This posterior perspective matters because it clarifies relationships that are easy to miss in an anterior heart view, especially the dominance of the left atrium and its close apposition to the esophagus and posterior mediastinum. That adjacency underpins common clinical problems: left atrial enlargement from mitral valve disease can compress the esophagus and distort the posterior wall, and radiofrequency ablation for atrial fibrillation targets tissue around the pulmonary vein ostia where iatrogenic pulmonary vein stenosis and, rarely, atrioesophageal fistula are feared complications. Coronary circulation is also readable from behind, with the coronary sinus coursing in the posterior atrioventricular groove toward the right atrium. A key landmark.
